When to Pay Your Bill
Your
payment is due and payable in full on the date indicated your invoice
from ITS. All non-disputed amounts are due and payable prior to
the generation of your next statement or you will be assessed a
late charge of $25.00. Finance fees of 1.5% will apply to all outstanding
balances and will begin accruing after 30 days.

How to Pay Your Bill
As
with everything ITS does, we offer our customers a number of options
to pay your bill so that you can choose the most convenient method
for you. Choose from one of the following options:
- • Automatic
payment plan using a credit or debit card -- ITS accepts MasterCard,
Visa, American Express, Discover and ATM. To set up your account
for automatic payments, please call us at 800-876-4ITS.
- • One-time
payment using a credit or debit card -- choose this option when
occasionally you want to pay your bill using your credit or debit
card. To pay your bill one time using your credit or debit card
call us at 800-876-4ITS.
- • CHAX
(Check by phone, fax, or email) – to utilize CHAX, please
call us at 800-876-4ITS to set up.
- • Mail
your payment using the remittance stub included in your bill.

Billing for Other Telephone Services
Should
you wish to dispute any charges billed to your telephone number
by a third party vendor, you must submit the dispute in writing
to ITS within 60 days from the receipt of the bill. Pending review,
you may withhold payment of the disputed amount only and collection
of those charges will be suspended. Should the third party vendor
agree to your dispute and issue a credit to ITS, we will pass these
credits through to your account. You have the right to dispute interstate
900 number calls that violate federal laws. In addition, you may
request blocking of 900 and 976 services.

Toll Free Service Disclaimer
Because
a toll free number is generally available to the public, ITS is
not responsible for misdialed numbers or incorrect advertising of
our client's toll free numbers by other persons or companies, and
credit will not be given for these calls.

Do-Not-Call Statutes and Regulations
As part of its compliance with Federal and State Do-Not-Call statutes and regulations, ITS would like to remind all clients of their responsibility to comply with these requirements.
All outbound telephone solicitations must comply with Federal Do-Not-Call statutes and regulations (47 C.F.R. 64. 1200, and 16 C.F.R. 310) and any applicable state laws. If you need assistance in understanding the legal obligations imposed in these state and federal laws, please contact your attorney for further guidance.

Disconnection of Service for Non-Payment
In
the event that your account is 60 days past due, a disconnect notice
will be sent to your billing address, and if the payment is not
received within 5 days after this notice has been sent, your service
will be disconnected. Should your account be disconnected, a reconnection
fee of $100.00 per line, plus any other related charges associated
with any special features or programming will be assessed and due
prior to re-establishment of service. Continued service by ITS shall
be subject to approval of the ITS Credit Department and may require
a deposit.

Subject: FCC Mandates “811” Service Nationwide by April
13, 2007
The
Federal Communications Commission (FCC) required, in CC Docket No.
92-105 (FCC 05-59), that all telecommunication carriers, including
wireline, wireless and payphone service providers implement the
8-1-1 service nationwide by April 13, 2007. FCC 05-59, implements
the Pipeline Safety Act, which establishes 811 as the toll-free
national abbreviated dialing code to be used by state One Call notification
systems for excavators and the general public to provide advanced
notice of excavation activities. Adoption of the dialing code will
enhance public safety and strengthen homeland security by streamlining
the notification process. Implementation of 811 nationwide will
improve public awareness through the use of a single dialing code
nationwide.
On
December 14, 2005, the Telecommunications Division (TD) of the California
Public Utilities Commission held its first informational workshop
regarding the implementation of the abbreviated dialing number 8-1-1
in response to the FCC 05-59. The workshop addressed the technical
and operational issues associated with the conversion from a 10-digit
telephone number to the use of a 3-digit N11 code. A second workshop
was held on July 27, 2006 where carriers were requested to discuss
issues regarding implementation of 811. TD staff specifically requested
discussions regarding the parties concerns with costs of implementation,
any anticipated challenges and the effective roll-out date if there
was one, prior to the mandatory date of April 13, 2007. Carriers
at the July workshop discussed the status of their implementation
strategies, timelines, costs, and charges associated with the new
dialing code. In this workshop the parties agreed to hold meetings
and negotiations to resolve any issue of the implementation costs.
The parties also agreed that there was no need for the Commission
to issue a resolution for the implementation of 8-1-1. One Call
Center (USA North and USA South) shared with carriers two 8YY numbers
to which carriers should route the 811 calls, based on the location
of the call. One call center has provided the toll-free number 1-866-888-7887
which connects to USA North and 1-888-300-5554 which connects to
USA South.
By
this letter, TD requests all carriers inform the CPUC of its effective
roll out date for 8-1-1 by March 1, 2007 and to file any related
tariffs or ICB contracts, as appropriate, reflecting the implementation
of 811. We would also ask you share with us any 8-1-1 availability
notices provided to customers, so that we may coordinate such information
with our Consumer Affair Branch in dealing with customer inquiries.
Since payphone providers are also required to provide 811 access
under this federal mandate, we request all payphone providers share
with us the status of their implementation efforts by March 1, 2007.

In
order to enhance public safety and homeland security, the Federal
Communications Commission has established a toll-free number to
dial to report any excavation activities that you might engage in.
Simply call 811 to report any plans to dig on or around your property.
This will ensure that no unknown underground cables are in the area
of planned excavation.

IRS Excise Tax Refund Info
Customers
who would like to receive a refund on their telephone Excise Tax
for Long Distance should go to IRS site at www.irs.gov. Instructions
are on first page of site. IRS Form 8913 should be filed with their
normal tax return.
As a courtesy to our customers, we are including
helpful tips on how to compute the percentage refund to be determined.
As specified in the IRS instructions, please compare your telephone
bills from April and September of 2006. Take the Federal Excise
tax for both months and compute a percentage of the excise tax to
the entire bill. Excise tax amounts should be decreased which allows
for a decrease in the percentage over that period. The difference
in the tax percentage should be applied to all telephone expenses
reaching back to March 1, 2003.
If Federal Excise tax has not been segregated in
April 2006, use the Federal Excise Tax Surcharges on that bill and
compare to the addition and combination on September of Federal
Excise Tax, Number Portability and Other Taxes and Surcharges. Compare
the difference in percents and apply to the form 8913.
As an example, if the April total bill is $1,000
and the Fed Excise Tax is $28, the percent of tax to bill is 2.8%.
Is September has a total bill of $2,000 and the Fed Excise is $38,
the percent of tax to bill is 1.8%. The excise tax has decreased
by 1.0% and this percentage should be utilized on the Form 8913
to compute the refunds going back to March 2003.
